Milestone 4.0.0

This will be the first version without Python 2 support.

My "must have" features for this release are ticket #110 (since we're already breaking backward compatibility be removing Python 2 support) and the test revision (ticket #98). After these two tickets are done, I'll consider whether I want to include more tickets for ftputil 4.0.0.
